  • Provide recommendations for improving reporting tactics and processes, to more effectively support ICP’s population health and quality initiatives

Responsibilities For Associate Population Health Director Resume

  • Work with ICP’s leadership to identify practice and physician needs and produce summary reports that help guide provider engagement and practice decisions
  • Effectively collaborate with team peers, other Provider Relations team members and partners throughout the system on an as needed basis
  • Administratively supports the CMO and Director of Population Health in driving Physician Engagement initiatives and activities that promote provider alignment and decrease burnout
  • Establishes individual annual goals
  • Support seamless flow of data from source to the front-end application
  • Play a critical role in driving success in population health performance by guiding care teams towards best practices, identifying gaps both through data and operational observation, by providing feedback and education, and by guiding actionable change from any gaps and root-causes identified
  • Guide and assist practice care teams to integrate and localize Evolent population health management tools and programs, including focus on risk adjustment and quality
  • Build trust-based relationships with medical practices and care teams to facilitate change management, engagement, solutioning, and sustainability of population health initiatives
  • Deliver and progress reports and review with medical practices and care teams. Identify performance gaps from report data and guide care teams towards solutions
