  • Collaborates with other Care Management team members as needed
  • Actively works to consolidate and eliminate redundancies within the 4 hospitals. Looks for ways to streamline efficiencies within the resource center to support care management in all hospitals and coordinates activities of the resource center (both current and planned) with
  • Able to analyze problems and deploy strategies to improve performance in collaboration with the leadership
  • National recognized Care Management certification within 5 years of start date (3 years for currently employed CCs)
  • Establish and maintain effective working relationships with employees, managers, healthcare professionals, physicians, and other members of senior administration, as well as the general public
  • Care management experience in a clinical managed care environment and knowledge of best practices
  • Demonstrated proficiency in intermediate level Word, Excel, Power Point, Email and Internet Explorer, as well as knowledge of electronic medical record systems
