  • Prescribes or refers for medical equipment, devices, physical and occupational therapy, and home health services per practice protocol
  • Identifies risk factors and behavior patterns that threatens health and well being; and educates patients and caregivers on ways to manage risk factors or modify behavior to avoid illnesses and maximize wellness
